Request Forms

Please note: We have updated our forms.  If you are submitting a parent paid request, we do NOT need an additional childcare or dual household request.  We just need the one, parent paid,  request for your student(s).
equest Types

There are four main parent request types; decline/reinstate busing request, dual household request, childcare provider request and parent paid contracts. Dual Household Requests and Childcare Provider Requests are free for qualified riders. If your child doesn't qualify for free transportation, you may want to consider applying for a Parent Paid Contract. If you are qualified for busing, but will not be using the ride OR if you have declined busing and would like to reinstate your ride, please fill out the Decline/Reinstate Busing Request. Requests are processed in the order they are received. Visit our Important Dates page for a list of request due dates.

Submit a Request Form

    Please note: When you submit your form, you will receive an e-mail confirmation. If you have a Google account, you will be able to view your request document. If you do not have a Google account, we will e-mail you a copy of your document upon request.

    Request Processing Timeline 

    We review routes in July and processing forms for each school as the routes are completed by the bus company. 

    Note: We will process every request in the order that we receive it. We, however, will not add new routes for parent paid contracts, so it is in your best interest to submit your form as early as possible.
